1.《境外人员体格检查记录表》须粘贴本人照片，加盖医院的公章并覆盖照片下沿，有效期为6个月。

Applicant’s photo is required on Physical Examination Record with the lower part of the photo stamped by the hospital. Physical examination results are valid for 6 months.

2.须在申请系统上传《境外人员体格检查记录表》、胸部X光片、心电图和特定项目的化验室报告等相关原始报告扫描件。

Please upload Physical Examination Record, as well as chest X-ray image, electrocardiogram, laboratory exam results and data sheets of some required items, into the application system. 

3.请使用我校表格进行体检，特殊情况如采用其他版本表格，检查项目以我校表格要求的为准。

Please conduct the physical examination with the XMU's form. In case of using forms of other institutes under special circumstances, please make sure all items required on the XMU's form are included.
